PDE5 inhibitors (tadalafil, sildenafil, vardenafil)
  • Do not take with nitrates (nitroglycerin, isosorbide) or recreational nitrites ("poppers") — combination can cause severe, potentially fatal low blood pressure.
  • Use caution with alpha-blockers (doxazosin, tamsulosin) and other blood-pressure medications — can cause lightheadedness or fainting.
  • Not appropriate if you have had a recent heart attack, stroke, or severe cardiovascular disease, or have uncontrolled hypertension or hypotension.
  • Seek emergency care for an erection lasting more than 4 hours (priapism) or sudden vision or hearing loss.
  • Common side effects: headache, flushing, mild nasal congestion, back pain, indigestion. Usually mild and transient.
Bremelanotide (MC4R melanocortin agonist)
  • Can transiently raise blood pressure and lower heart rate. Not appropriate if you have uncontrolled or high-risk cardiovascular disease.
  • Most common side effect: nausea, especially on the first dose. Taking with food can reduce nausea.
  • Can cause focal darkening of the skin (melanocyte stimulation) with frequent use — usually reversible.
  • Do not use more than one dose within 24 hours or more than 8 doses per month.
  • Disclose all medications at intake, including blood-pressure medications.
Pycnogenol (botanical)
  • May have mild blood-thinning effects. If you take warfarin, heparin, aspirin, clopidogrel, or other anticoagulants, tell your prescriber.
  • Discontinue at least 2 weeks before scheduled surgery.
